  • Coordinate interdisciplinary team efforts to deliver seamless hospice services across various settings including assisted living facilities, nursing homes, home health environments, and home care.
  • Monitor shift management to ensure appropriate staffing levels that support quality patient care while maintaining operational efficiency.
  • Conduct regular audits of medical records and documentation to uphold standards in medical terminology accuracy and compliance with federal regulations.
  • Provide leadership in nurse management activities such as supervising staff performance, facilitating training on medical records systems, EMR/EHR systems, and ensuring staff are well-versed in hospice-specific procedures.
  • Manage the review of patients (personalized care needs), ensuring that all aspects of their comfort, symptom control, and emotional support are addressed effectively.
  • Stay current on healthcare regulations including Medicare guidelines related to hospice services to ensure ongoing compliance and reimbursement accuracy.

Skills

  • Management skills with supervising experience in nursing administration or nurse management roles.
  • Strong knowledge of MDS (Minimum Data Set) assessments used for resident evaluations in long-term care settings.
  • Proficiency with EMR (Electronic Medical Records) systems and EHR (Electronic Health Record) systems for documentation review and record keeping.
  • Deep understanding of physiology and anatomy relevant to end-of-life care to inform effective treatment planning.
  • Excellent leadership abilities with a track record of supervising teams effectively while fostering professional development.
  • Familiarity with shift management strategies to ensure optimal staffing coverage at all times.
  • Knowledge of HIPAA regulations governing patient privacy and confidentiality across all healthcare documentation processes.
  • Ability to communicate medical terminology clearly with interdisciplinary teams while maintaining compassionate patient interactions.
