PRN Home Health and Therapy is Hiring a Patient Intake supervisor. This position will supervise the intake department, handle scheduling. Schedule of M-F 8-5 in our Appleton Office

Responsibilities include:
- Oversee the day to day operations of the intake team
- Schedule patient visits with clinicians
- Ensure effective and timely coordination of patient home health services
- Assist as needed in calls to patients, caregivers, providers, facilities to coordinate and oversee patient care
- Conduct monthly check-ins to ensure best patient experience
- Ensure seamless transition of patients to homecare by providing direct oversight of patient education, preparation for homecare, and coordination of care with multiple service providers
- Update supervisor on any concerns, constrictions, ideas to improve processes
- Promote the programs and services of PRN Home Health and Therapy through formal and informal interactions with the community
Requirements:
- Licensed in Wisconsin as Licensed Practical Nurse (preferred), COTA, PTA, or significant relevant job experience
- One year of clinical experience in healthcare settings(Preferred)
- Excellent verbal and written communication skills
- An ability to work individually or in a team environment; Self-directing with the ability to work with little direct supervision
- Proficiency with Microsoft Office Suite
- Good judgement and discretion when dealing with sensitive topics
- Strong computer and technology skills
- Able to supervise others in a team-oriented environment
