Job Summary:
We are looking for a highly compassionate and dedicated RN to join our growing team. You must be licensed to work in the state of Georgia, to conduct initial client assessments, develop personalized care plans, and perform mandated in-home supervisory visits. This is a field-based position designed for a nurse who enjoys building relationships with clients and ensuring high-quality care in the comfort of their homes.
Responsibilities & Duties:
- Initial Assessments: Conduct comprehensive in-home head to toe physical assessments and determine care needs.
- Care Planning: Develop, review, and update individualized Plans of Care in accordance with Georgia state regulations.
- Observe and report symptoms, reaction to treatments, drugs, and changes in the client's physical or emotional condition.
- Initiate preventative procedures as appropriate for the client's care and safety and effecting changes in environment to ensure client safety and security.
- Coordinate services for clients as needed to other agencies and health care providers as appropriate.
- Provide supervision of caregiver services and preparing written instructions for care provided by aides.
- Supervisory Visits: Perform quarterly (or more frequent) in-home supervisory visits to evaluate caregivers (CNAs/PCAs) and assess client satisfaction/safety.
- Documentation: Complete required clinical documentation and ensure compliance with Georgia Department of Community Health (DCH) regulations.
- Caregiver Education: Provide guidance, training, and competency evaluations for field staff.
- Communication: Act as a liaison between clients, family members, and the agency office regarding client status changes.
- Maintain confidentiality of patient and agency matters.
- Submit required documentation in a timely manner.
- Report observed or suspected abuse pursuant to mandated requirements.
- Assist the Administrator in decision making and policy formation as it pertains to the Home Care Services.
- Participate in surveys, studies and special projects as assigned.
- Participating in weekend/holiday call schedule, as needed.
- Observe agency’s attendance requirements and dress code.
- Conserve agency resources and adequately maintaining agency property, supplies, and equipment.
- Adhere to all agency policies, including but not limited to, infection control and safety, education, reporting and practice implementation.

Qualifications & Education:
- Graduate of an accredited school of nursing
- Current and active Registered Nurse (RN) license in the state of Georgia.
- Strong understanding of GA Private Home Care Provider rules (Chapter 111-8-65).
- BLS Certificate
- TB screening test result within one year
- 1-3 years of nursing experience (home care experience, preferred)
- Supervisory skills, a plus
- In-home care new client assessment experience preferred.
- Excellent communication skills, flexible and able to interact positively with the rest of the staff.
- Strong sense of initiative and urgency in addressing unexpected patient crises
- Self-directed, able to work without presence of immediate supervisor.
- Commitment to regulatory compliance and to submitting all documentation in a timely fashion
- Demonstrated knowledge regarding exposure to body fluids and chemicals, as well as to some hazardous equipment
Must have the ability to:
- See and hear adequately to respond to auditory and visual requests.
- Pass a background screen.
- Travel as required and be in possession of a valid driver’s license and automobile insurance.
