We here at 1st Option Adult Day Services & Home Health are looking for an attentive and knowledgeable home health nurse who will assist in ensuring the nursing department provides consistent, attentive, and efficient care to patients that are in home. The nurse is responsible for monitoring nursing staff, nursing activities, and patient care, as well as ensuring nursing activities are in compliance with federal, state, and local standards. Responsible, attentive, knowledgeable, and provides high quality care to patients. You should provide support to nursing staff and the director of nursing, and thoughtful care to patients.

The position of a nurse demands a high level of clinical expertise combined with strong interpersonal skills. They must be adept at assessing various medical conditions while also providing emotional support to patients and their families during what can often be a stressful experience. Their role is integral to maintaining patient flow efficiency and ensuring that subsequent care providers have all the necessary information to deliver optimal care. In essence, the Admission Nurse is a pivotal figure in fostering a smooth and effective entry into the healthcare system, contributing significantly to both patient outcomes and institutional efficiency. Assume primary responsibility for clinical services provided by the Agency. Direct, coordinate and supervise all clinical functions. Maintains compliance with federal and state regulations as they apply to the provision of home visits to patients.

Responsibilities:

• Patient Assessment: Nurse conduct thorough assessments, including gathering medical history, vital signs, and other relevant information.
• Paperwork and Documentation: Nurse is required to work as well in the office and they ensure all necessary admission paperwork is completed accurately and efficiently, including insurance information and consent forms.
• Patient Education: Nurses provide patients and their families with essential information about the admission process, their rights, and what to expect during their stay.
• Care Coordination: They coordinate with other healthcare professionals, such as physicians and specialists, to ensure patients receive appropriate care.
• Transition Management: Nurses play a key role in facilitating the transition of patients to different care settings, such as from the emergency room to a hospital room or from the hospital to a rehabilitation facility.

Nursing Requirements:

• A Bachelor's or Associate's degree in Nursing.
• A Registered Nursing license.
• Experience in Nursing in Home.
• Good critical thinking, communication, and leadership skills.
• Good planning and problem-solving skills.
• Knowledge of computers and healthcare software.
