JOB SUMMARY

Under the direction and supervision of the Home Care Coordinator, RN the Home Health Aide is responsible for providing and assisting participants with activities of daily living and supportive care necessary to the preservation of the home environment. Assists in monitoring the health status and providing a safe environment for participants. Functions as a member of the multidisciplinary team. Promotes professional working relationships with both internal and external customers. Adheres to and supports all organizational policies and procedures and standards. Promotes teamwork!

ESSENTIAL DUTIES AND RESPONSIBILITIES

ORGANIZATIONAL (CORE RATING FACTORS):

  • Ensure discretion with confidential information.
  • Maintains courteous, helpful and professional behavior on the job. Will support the success of the entire team by promoting a collaborative work environment.
  • Adheres to all Policies and Procedures, Safety Policies and Procedures and OSHA safety guidelines.
  • Consistently contributes to problem-solving and cooperates with identified resolutions.
  • Must demonstrate regular attendance and punctuality.
  • Brings issues and process improvement ideas to the attention of the Supervisor.
  • Maintains verbal and written skills required for the position.
  • Attends meetings and participates in committees as required.
  • Completes annual training as required.
  • Adheres to PACE of West Baltimore Dress Code Policy.
  • Demonstrates appropriate level of time management in support of co-workers and the entire team.

DEPARTMENTAL (JOB SPECIFIC TASKS):

  • Must maintain calm and professional demeanor.
  • Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.
  • Assists in monitoring participant’s health status (vital signs, weight, skin condition, appetite and changes in functional ability). Observes participant’s taking prescribed medications. Reports pertinent information to Home Care Coordinator in a timely manner.
  • Assists participants with ADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) as assigned.
  • Documents services provided to participants consistently and accurately via Electronic Health Record (EHR). Follows procedure for flow sheet completion. Documentation is completed in a timely manner.
  • Participates in participant’s care planning through cooperation with multidisciplinary teams.
  • Participates in Quality Assurance programs as assigned
  • Handles potentially infectious specimens with appropriate biohazard precautions and practices universal precautions.
  • Ensures customer satisfaction.
  • Participates in Company required and assigned training programs and any required staff and training meetings.
  • Responsible for checking Company email at least once per week to receive updated scheduling information.
  • Maintains proper Boundaries when dealing with participants. Notifies supervisor of any possible ethical or boundary issues immediately upon discovery.
  • Responsible for maintaining professional affiliations and any required certifications.
  • Additional responsibilities will be added at managers discretion, as career growth opportunities and responsibilities are constantly arising in our flexible and start-up environment.

QUALIFICATIONS

  • High school diploma or equivalent.
  • Must be an active Certified Nurse Assistant (CNA) in Maryland.
  • Must have a driver's license and reliable transportation
  • Minimum of one year of experience with a frail or elderly population required.
  • Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers.
  • Must submit to and pass a pre-employment drug/alcohol screening and criminal background check.
  • Must have a current Driver’s License and driving record must be in compliance with Company policy. Must maintain current automobile insurance.
  • Must be clear of all communicable diseases and have all immunizations up to date before providing direct care.
  • Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records.
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public.
  • Must be free of communicable diseases that can be readily transmitted in the workplace. TB test is required within the first 7 days of employment. HepB vaccinations are offered but not required.
  • Operates in full compliance with all applicable federal, state and local fire, health, safety, sanitation, Emergency procedures and other Standards prescribed in law or regulations.
  • Ability to work independently with minimum supervision.
  • Knowledge of how to use and maintain in-home equipment (adaptive, hoyerlift, etc.).

  • Visual, Hearing and Communication Requirements: Requires corrected vision and hearing to within normal range, with or without reasonable accommodation. Must be able to communicate effectively in verbal and written form with all levels of personnel within and outside of the organization.   
  • Pressure Factor: Requires working under stressful conditions. Moderate pressure to meet scheduled and recurring deadlines.

  • Environmental Conditions: Working conditions may be noisy with fluctuating indoor temperatures.  Must have the ability to work under moderate pressure in order to meet scheduled appointments or deadlines while dealing with frail, disabled and/or confused participants/clients. Subject to individuals who may have the potential for physical or verbal aggression. May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases, and other conditions common in a clinic environment.
