Schedule:
Day Shift: 8:00 AM – 4:30 PM
Rotates every 4th weekend and holiday.
Position Overview:
EverHeart Hospice is seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join our dedicated care team. In this role, you will provide high-quality, hands-on patient care and support to individuals and families during daytime hours.
Responsibilities:
Responsibilities of the Position:
- Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs, including:
- Routine Home Care Visits
- End of Life / Continuous Care
- Referrals
- Care Center Assistance
- Chart Reviews
- Other duties as assigned
- Recognize and promptly report any changes in the patient’s mental and/or physical status to the registered nurse, both verbally and in writing when appropriate
- Explain hospice services and benefits related to Medicare, Medicaid, and private insurance; understand when to implement Palliative Care interventions and use ABNs
- Obtain and enter orders from Medical Directors and Physicians when appropriate
- Order necessary Durable Medical Equipment (DME) for patients.
- Facilitate continuity of care by coordinating the admission of home-care patients to inpatient settings (hospital, nursing home), as needed.
- Work evenings and weekends as needed to meet patient, family, and agency scheduling needs
- Complete all documentation in accordance with agency policy, accreditation standards, and governing requirements
- Maintain clinical competency for assigned responsibilities and procedures
- Perform other duties as assigned
Qualifications:
Education and Experience:
- Graduation from an approved School of Nursing
- Current LPN licensure in the State of Ohio
- Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing
Special Skills and Requirements:
- Shall have a valid driver’s license
- Excellent customer service skills
- Detailed with excellent communication, documentation, and organization
- Ability to follow instructions
- Ability to work independently without supervision
- Travel to other sites as needed. Travel may be in inclement weather
- Ability to lift 25 lbs of supplies or equipment
- Requires various amounts of sitting, standing, twisting, and turning, bending, lifting, and walking
- Ability to assist with patient care activities such as ambulation, transfers, and positioning
