Care Coordinator

Help vulnerable Medicaid members access healthcare and life-changing services.

As a Care Coordinator with Bridging Care, you will guide members through the healthcare system and connect them with medical, behavioral health, and community resources.

CMA certified? Your clinical foundation is exactly what Health Home care coordination needs — especially if you have field, FQHC, mobile clinic, or home health experience. This is a role that pays more and means more.

This is not a clinical nursing role. Your MA training is the foundation — not a ceiling.

Washington State's Health Homes program includes a Tier 3 qualification specifically for Certified Medical Assistants (CMA/RMA) who hold an Associate Degree. That is you. Health Home Care Coordinators do not administer medications or perform clinical procedures — they build care plans, coordinate across providers, and show up for members who have nowhere else to turn.

If you have been working as a CMA in a clinic setting and want a role that pays more, offers more flexibility, and puts you directly in the community — this is a career move, not a step sideways.

CMA / RMA with an Associate Degree qualifies at Tier 3. CMAs with experience in field settings — FQHC, home health, mobile clinic, or managed care — are particularly strong candidates. Clinical setting MAs are also welcome. If you have community-facing experience and a passion for the population we serve, your background translates directly.

The Work

What you will actually be doing.

  • In-home and community visits. You go where your members are. You build the kind of relationship no clinic appointment allows.
  • Health Action Plan development. With each member, you build a personalized plan addressing their chronic conditions, goals, and real-world barriers.
  • Cross-provider coordination. You are the connective tissue — making sure the member's doctors, behavioral health providers, housing supports, and social services are aligned.
  • Telehealth visits and documentation. Tier 2 and Tier 3 visit types in addition to in-person field work. Comprehensive training on all systems before you carry a live caseload.

You will always know what success looks like. Clear goals, daily supervisor check-ins in Month 1, a 4-week training program, and a mentor CC who shadows your first real visits. You are never thrown in the deep end.
