We are seeking a licensed Psychiatric Mental Health Nurse Practitioner to provide ongoing psychiatric services within residential and post-acute care environments. This role is well-suited for a mission-driven clinician who is passionate about improving access to mental health care for an underserved adult and geriatric population. The position offers a consistent schedule, meaningful patient relationships, and the ability to practice independently in a non-acute setting.
Requirements
- Perform comprehensive psychiatric evaluations and deliver longitudinal follow-up care
- Oversee psychotropic medication management and monitor clinical outcomes
- Develop and refine individualized treatment plans aligned with patient needs
- Partner closely with nursing staff, facility leadership, and interdisciplinary teams
- Communicate clearly and compassionately with patients and involved family members
- Deliver psychiatric care using patient-centered and trauma-informed principles
- Maintain accurate, timely documentation in accordance with regulatory standards
Minimum Requirements
- Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Active, unrestricted state licensure
- Strong clinical judgment, communication, and interpersonal skills
- Ability to function independently while contributing to a collaborative care model
Preferred Experience
- Experience working with geriatric patients or within long-term or skilled nursing settings
- Prior background as a psychiatric RN or NP in institutional or residential environments is a plus
