Now Hiring: Permanency Specialist | Permanency Worker

Location: Berks County & Surrounding Counties, PA 19606

Pay: $20.00 per hour

The Children's Home of Reading Youth and Family Services is seeking a Permanency Specialist for our Adoption Program, servicing the Berks and surrounding counties. Position provides SWAN adoption services to children and families preparing them for permanency.

Job Type: Full-time

As a Permanency Specialist, you will help families and children achieve permanency, which can include reuniting with their biological family or finding a permanent placement. In this role, you must have strong organizational, writing, and verbal skills.

Requirements:

  • Bachelor's Degree in social work, psychology, or human services field REQUIRED.
  • Experience with children, youth and families preferred.
  • One year verified experience in SWAN adoption work preferred.
  • Must be available and willing to work a flexible schedule.
  • Must hold a valid PA driver's license and a good driving record, with proof of auto insurance coverage. Must hold own liability insurance.
  • Must be at least 21 years old.
  • Bilingual a plus

Responsibilities may include, but not limited to:

  • Case management: Creating and implementing case plans for children and their families, and supervising their progress
  • Family support: Working with families to strengthen their unit and keep children safe
  • Service coordination: Providing and coordinating services for families, and ensuring they have access to service providers
  • Foster care visits: Visiting children in foster care facilities at least once every 30 days
  • Written reports: Preparing written reports for leadership teams on topics such as client safety and wellbeing
  • Data collection: Gathering statistical data on client information and goal achievement
  • Problem solving: Serving as a first line of defense for problem solving and de-escalation in crisis situations
  • Staff supervision: Supervising assigned staff, including other permanency workers and specialists

Additional Requirements: The candidate must pass required Criminal Record History Clearance, FBI and Child Abuse History Clearance, as well as possess a valid PA Driver's License. The candidate must pass a Physical & Drug Screening & possess the ability to attend and successfully complete all required trainings
