Job Title: Licensed Master Social Worker
Location: New York, NY 10016
Pay Rate: $2200/ - $2400/weekly, $60/hr - $62/hr
Duration: 3 months + probable extension
Schedule: Monday - Friday, 9:00 AM – 5:00 PM (35 hrs/week)
Requirements:
Minimum 1 year of LMSW experience - must be recent
Hospital/Inpatient experience (MANDATORY) - must be recent
Inpatient medicine, surgery, (med/surg), rehab experience - Required
Master’s degree in social work
Valid NYSED LMSW License
Duties:
Conduct psychosocial assessments to identify patients’ social, financial, and environmental needs
Assist clients with care coordination, discharge planning, and community resource referrals
Provide case management support, including follow-ups and service linkage
Advocate for patients regarding housing, insurance, benefits, transportation, and social services
