We are seeking a compassionate and dedicated CNA & CHHA's for the OKC Metro area to travel to patient's home's and provide essential care to our patients mainly DAYS & SATURDAY MORNINGS with occasional evening shifts available. This role is vital in ensuring the comfort and well-being of our residents, particularly those in long-term care, assisted living, and memory care settings. The ideal candidate will have a strong background in patient care and experience working with diverse populations, including individuals with developmental disabilities and those requiring specialized care such as Alzheimer's and dementia support.

Responsibilities

• Assist patients with daily living activities, including bathing, dressing, grooming, and feeding.
• Monitor and record vital signs, ensuring accurate medical documentation.
• Provide companionship and emotional support to residents.
• Administer medications as prescribed and assist with medication management.
• Utilize medical equipment such as Hoyer lifts safely and effectively.
• Maintain a clean and safe environment for patients while adhering to infection control protocols.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Document patient progress and changes in condition using EMR/EHR systems.
• Uphold HIPAA regulations to protect patient privacy.

Requirements

• Current CNA and/ or CHHA certification is required.
• Must have reliable transportation.
• Current Driver’s License and Insurance is required.
• Experience in long-term care, assisted living, or nursing home settings is preferred.
• Familiarity with medical terminology, anatomy knowledge, and physiology knowledge is essential.
• Previous experience in critical care, hospital medicine, or emergency medicine is a plus.
• Strong communication skills with the ability to work effectively within a team environment.
• Knowledge of basic life support techniques and phlebotomy is advantageous.
• Experience working with individuals with developmental disabilities or behavioral health needs is beneficial.
