- Evening: 3:00 PM – 11:00 PM
- Day: 7:00 AM – 3:00 PM
Schedule: 40 hours/week
Weekend/Holiday Requirement: Every other weekend and holiday
Job Summary
"Navitas Healthcare, LLC" is a Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The LPN will deliver direct patient care, supervise nursing assistants, and ensure compliance with healthcare regulations while promoting resident well-being and safety.
Key Responsibilities
- Provide direct nursing care in accordance with individualized care plans
- Supervise and guide nursing assistants during assigned shifts
- Administer medications and treatments as prescribed
- Transcribe physician orders into charts, medication records, and care plans
- Monitor resident conditions and document nursing notes accurately
- Assist with admissions, transfers, and discharges
- Communicate with physicians regarding care plans and changes in condition
- Perform clinical procedures such as catheterization, tube feeding, suctioning, and wound care
- Collect specimens (urine, blood, sputum) for testing
- Record vital signs including temperature, pulse, respiration, and blood pressure
- Participate in care planning and interdisciplinary team collaboration
- Provide education to residents and families
- Ensure adequate supplies and medications on the unit
- Maintain patient confidentiality and adhere to HIPAA guidelines
- Support audits, surveys, and quality improvement initiatives
- Assist with orientation of new staff and residents
- Follow infection control and safety protocols
- Deliver excellent customer service to residents, families, and staff
Qualifications
- Graduate of an approved LPN program or accredited nursing school
- Active Massachusetts LPN license required
- BLS / CPR / AED certification required
- Ability to communicate effectively in English
- Experience in long-term care or skilled nursing preferred
- Strong clinical and documentation skills
