Segue Health, a faith-based medical practice, is seeking an experienced and compassionateNurse Practitionerto join our team serving patients inCrowley, Wedgwood, TCU, and surrounding areas. This role focuses on providingin-home post-discharge care management and medical treatmentfor patients transitioning from acute care facilities back to their homes.
Key Responsibilities
- Conduct in-home patient visits to assess clinical status and provide post-discharge medical care.
- Review and interpret discharge summaries and progress notes.
- Collaborate closely with home health professionals and other members of the care team to ensure seamless coordination of care.
- Provide medical consultations, prescribe medications when appropriate, and oversee ongoing care plans to promote recovery and reduce readmissions.
Qualifications
- CurrentTexas medical licenseand activeMedicare provider numberrequired.
- Nurse Practitioner certification in good standing.
- Ability to travel within a35-mile radiusfrom home to see patients throughoutCrowley, Wedgwood, TCU, and surrounding areas.
- Strong communication, clinical judgment, and documentation skills.
- A compassionate, faith-driven approach to patient care.
Compensation:
- Base Salary:$115,000 $135,000 (commensurate with experience)
- Quarterly Performance Bonusesavailable
