Salary Range:
$42.64 - $65.01 per hour

Key Responsibilities:

  • Accurately record and maintain patient care referral data from multiple sources, both digitally and in log books.
  • Build a strong understanding of insurance programs (Medicare, private insurance, HMO, Title 19) to make informed decisions about care eligibility and acceptance.
  • Ensure smooth transitions from hospital to home by collaborating with hospital staff, patients, and families to review and assess home care referrals.
  • Communicate new referrals promptly to the appropriate program supervisor (Home Health Nursing, HHA, Therapy, Maternal Child Health, Social Work).
  • Oversee medication reconciliation, manage medical equipment orders, and provide essential patient education throughout the discharge process.
  • Ensure patients and families are well-prepared for the next steps in their home care journey.

Requirements:

  • RN License in Connecticut.
  • Knowledge of managed care operations, Medicare and Medicaid and other payer sources. Deals with a high degree of public contact and interactions.
  • One year home health or hospital case management/discharge planning

Work Type: Full-Time
Schedule: Monday to Friday, Day Shift (with occasional rotation)
