Job Summary
The Case Manager for Senior Services provides comprehensive case management and supportive services to older adults in New York City. This role focuses on assisting seniors to maintain independence, stability, and quality of life by connecting them to essential social, medical, housing, and financial resources. The Case Manager works closely with clients, families, healthcare providers, and community agencies to develop and implement individualized service plans that address the unique needs of the aging population.
Key Responsibilities
- Conduct comprehensive psychosocial assessments to evaluate seniors’ needs, strengths, and barriers related to health, housing, finances, and social support.
- Develop, implement, and monitor individualized service plans to support aging in place and overall well-being.
- Coordinate and link clients to community-based resources, including home care services, benefits and entitlements (Medicaid, Medicare, SNAP, SSI), transportation, meal programs, and senior centers.
- Advocate on behalf of clients to ensure access to appropriate medical care, mental health services, and long-term support services.
- Provide ongoing case management, follow-up, and crisis intervention as needed.
- Collaborate with healthcare providers, Adult Protective Services (APS), housing agencies, and other community partners.
- Assist seniors and families with navigating complex systems and completing applications and documentation.
- Maintain accurate, timely, and compliant case notes and records in accordance with agency, city, and state regulations.
- Participate in interdisciplinary team meetings, trainings, and case conferences.
- Ensure services are delivered in a culturally competent, trauma-informed, and client-centered manner.
Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, Gerontology, or a related field required; Master’s degree (MSW) preferred.
- Minimum of 1–2 years of experience working with seniors, case management, or community-based social services.
- Knowledge of New York City and New York State aging services, entitlement programs, and community resources.
- Strong assessment, documentation, and organizational skills.
- Excellent interpersonal, communication, and advocacy skills.
- Ability to work independently and manage a caseload in a fast-paced environment.
- Proficiency with electronic case management systems and Microsoft Office.
