Summary:

The Nurse at Revive will provide compassionate care and support to individuals facing challenges related to substance use or mental health. The role emphasizes holistic approaches to recovery, in line with Revive's mission to empower individuals towards a life free from substance abuse.

Essential Duties and Responsibilities:

  • Provide patient-centered care and support in alignment with Revive's holistic recovery programs.

  • Record and communicate treatment activities in accordance with the organization’s policies and procedures.

  • Administer prescribed treatments and monitor patients' medication usage and results.

  • Conduct thorough patient assessments and document findings accurately.

  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.

  • Educate patients and their families about treatment plans, procedures, and ongoing care, emphasizing the principles of Revive's 12-Step Program.

  • Ensure all activities comply with healthcare regulations and Revive's standards of care.

  • Maintain a clean and safe clinical environment.

  • Participate in continuous improvement initiatives and staff training sessions.

  • Provide support during outpatient programs as needed and for all levels of support throughout the facility.

  • Monitor the use and status of medical and pharmaceutical supplies.

  • Other duties as assigned.

Supervisory:

This position does not oversee staff.

Education/Experience

  • Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.

  • Minimum of 2 years of experience in a clinical setting, preferably in substance abuse treatment or behavioral health.

  • Excellent organizational skills and attention to detail.

  • Strong verbal and written communication skills.

  • Ability to work effectively in a team environment.

  • Compassionate and empathetic approach to patient care.

  • Ability to handle sensitive information with confidentiality.

  • Excellent documentation skills

  • Experience with electronic health records (EHR)

Certificates, Licenses, Registrations

Current Licensed Practical Nurse (LPN) license.

Work Environment:

The work is typically performed in a clinical setting with moderate noise levels. This position requires frequent interaction with patients and the public. Occasional exposure to hazardous materials and sharp objects.

Physical Demands:

  • Prolonged periods of standing and walking.

  • Must be able to lift up to 30 pounds regularly, and up to 50 pounds on occasion.

  • Ability to perform repetitive movements over long periods.

  • Ability to handle fast-paced situations, especially during patient crises.
