Location: Panama City, FL (Onsite)
Schedule: Day Shift | Weekdays
Hours: 7–10 hours per day
May include occasional on-call rotations, late evenings, or extended days as needed

Position Summary

"Navitas Healthcare, LLC" is seeking Director of Case Management for an exciting job in Panama City, FL.

Key Responsibilities

• Provide overall leadership and direction for case management and social work operations.

• Oversee departmental structure, staffing models, onboarding, competencies, and performance management.

• Ensure effective allocation of resources to meet productivity, quality, and operational goals.

• Maintain oversight of core case management functions, professional practice standards, and documentation accuracy.

• Ensure compliance with care coordination, discharge planning, and regulatory requirements.

• Collaborate with finance and senior leadership to support operational and financial objectives.

• Analyze performance metrics, identify opportunities for improvement, and implement sustainable process enhancements.

• Build strong working relationships with executive leadership, nursing, physicians, ancillary services, ethics, compliance, and legal teams.

• Lead change management initiatives and foster a collaborative, high-performing team culture.

• Perform additional duties in alignment with organizational mission, values, and code of conduct.

Required Qualifications

• Minimum 3 years of recent acute care hospital case management experience (within the past year)

• Minimum 2 years of leadership or management experience

• Bachelor’s degree (required)

• Active licensure as one of the following:

  • Registered Nurse (RN)

  • Licensed Clinical Social Worker (LCSW)

  • Licensed Master Social Worker (LMSW)

Preferred Qualifications

• Master’s degree in:

  • Nursing

  • Health Administration

  • Business Administration
