RN Case Manager Hours
- Monday to Friday 8am - 5pm
- County Covered: Washington County/Chisago County/Anoka County
RN Case Manager Compensation
- The pay for this position is $50.00-55.00 an hour paid weekly
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN Case Manager Responsibilities
- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Continuously evaluate appropriate, cost-effective levels of care
- Coordinate care transitions with physicians, social workers, discharge planners, and claims teams
- Arrange non-medical support services (e.g., housing, transportation) to support treatment adherence
- Engage specialty resources as needed to improve patient outcomes
- Maintain detailed documentation of clinical, functional, and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate while ensuring privacy and confidentiality
- Intervene promptly to prevent or minimize adverse patient outcomes
RN Case Manager Requirements
- Licensed RN
- Home Health/Hospice experience or case management highly preferred
