Responsibilities
- Administer prescribed medications and monitor residents per care plans
- Conduct nursing assessments, document vital signs, and report changes to the care team
- Provide direct patient care, including assistance with activities of daily living
- Collaborate with the registered nurse and interdisciplinary team to implement care plans
- Complete accurate charting in the electronic health record and maintain MAR updates
- Respond to call bells and emergencies; uphold safety, infection control, and wound care per protocol
Requirements
- Active Licensed Practical Nurse license in Vermont or eligible for licensure in Vermont
- Graduation from an approved practical nursing program (LPN diploma)
- Ability to work evening and night shifts
- BLS certification required
- Strong communication and documentation skills; proficient with electronic health records
- Long-term care experience preferred but not required
