Contract: 13 Weeks
Shift: Days | 5x8 (8:00 AM – 4:30 PM)
Requirements
- Active VA PT License or Compact License
- BLS required
- Minimum 1 year Home Health experience (post-graduation)
- OASIS proficiency required
- EPIC experience preferred
Responsibilities
- Provide physical therapy services in home health settings
- Complete evaluations, reassessments, start of care, and follow-up visits
- Maintain accurate OASIS and patient documentation
- Manage approximately 30 productivity points per week
- Collaborate with interdisciplinary healthcare teams for patient care planning
