Job Description

Join Our Team at Sunbright Home Health & Hospice!
We’re seeking a dedicated and compassionate Physical Therapist to serve patients in the Bay Area and Central Valley. As part of our skilled clinical team, you’ll help patients regain strength, mobility, and independence in the comfort of their own homes. If you’re passionate about providing quality care and making a meaningful difference every day, we’d love to hear from you!

About the Role:

As a Physical Therapist in California's dynamic healthcare environment, you will play a critical role in improving patients' physical function and quality of life through personalized rehabilitation programs. You will assess, diagnose, and treat individuals with a variety of musculoskeletal, neurological, and orthopedic conditions, including sports injuries and geriatric rehabilitation needs. Your expertise will guide patients through manual therapy techniques and exercise regimens designed to restore mobility, reduce pain, and prevent further injury. Collaborating closely with physicians, occupational therapists, and other healthcare professionals, you will develop comprehensive care plans tailored to each patient's unique goals and medical history. Ultimately, your work will empower patients to regain independence and enhance their overall well-being through evidence-based physical therapy interventions.

Minimum Qualifications:

  • Licensed Physical Therapist in the state of California.
  • Degree in Physical Therapy from an accredited institution (Doctor of Physical Therapy preferred).
  • Demonstrated experience in orthopedic rehabilitation and manual therapy techniques.
  • Strong knowledge of anatomy, physiology, and kinesiology relevant to physical therapy practice.
  • Excellent communication and interpersonal skills to effectively interact with patients and healthcare teams.

Preferred Qualifications:

  • Certification in Sports Medicine or Orthopedic Physical Therapy.
  • Experience working with geriatric populations and managing age-related rehabilitation challenges.
  • Familiarity with electronic medical records (EMR) systems and healthcare documentation software.
  • Advanced training in specialized manual therapy approaches such as Maitland or Mulligan techniques.
  • Bilingual abilities to serve diverse patient populations within California.

Responsibilities:

  • Conduct thorough patient evaluations to assess physical abilities, limitations, and rehabilitation needs.
  • Develop and implement individualized treatment plans focusing on orthopedic rehabilitation, sports medicine, and geriatric care.
  • Apply manual therapy techniques and therapeutic exercises to improve patient mobility and reduce pain.
  • Monitor patient progress and adjust treatment plans accordingly to ensure optimal recovery outcomes.
  • Educate patients and their families on injury prevention, home exercise programs, and lifestyle modifications.
  • Collaborate with multidisciplinary healthcare teams to coordinate patient care and share clinical insights.
  • Maintain accurate and detailed patient records in compliance with healthcare regulations and standards.

Skills:

The required skills such as physical therapy, orthopedic rehabilitation, and manual therapy are essential for conducting patient assessments and delivering targeted treatment plans that address specific injuries and conditions. Proficiency in sports medicine and managing sports injuries enables the therapist to support athletes in recovery and performance optimization. Geriatric rehabilitation skills are applied to tailor interventions that accommodate age-related physical limitations and promote functional independence. Collaboration and communication skills are vital for coordinating care with other healthcare professionals and educating patients effectively. Preferred skills like advanced certifications and bilingualism enhance the therapist's ability to provide specialized care and serve a broader community, improving patient outcomes and satisfaction.
