Description

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients under the supervision of registered nurses and physicians. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care.

Duties And Responsibilities

  • Responsible with Care Team members for pre-visit planning to ensure required screenings, laboratory testing, and immunizations are up to date
  • Assists with physical exams, completes required screenings, and laboratory testing
  • Administers treatments, immunizations, medications to patients as ordered by the provider
  • Provides patients services, educate patient/parent as needed, assists the patients with prescription needs/refills, coordinates services with and for the patient, acts as a client advocate to pursue financial assistance, and initiates referrals to community-based organizations as needed
  • Responds to calls from patients, pharmacies, home care agencies, and family members; collaborates with school nurses, WIC colleagues, DCF, and other medical departments as needed, and completes paperwork needed by external organizations

Requirements

Education and Experience

  • High School degree and LPN training course
  • One to three years of experience in a healthcare setting with prior experience in ambulatory care strongly preferred

Certificates, Licenses, Registrations

  • Active Connecticut LPN license
  • Current CPR certificate

Other Skills And Abilities

  • Strong clinical skills and knowledge of nursing practices.

  • Compassionate and patient-centered approach to care.
  • Proficient in electronic health record (EHR) systems with working knowledge of MS Office
