Lakeshore Talent is partnering with a mission-driven organization dedicated to supporting individuals with disabilities and those in need of long-term care services in their search for an Intake Case Manager.
The Intake Case Manager plays a vital role in helping individuals access essential supports by conducting functional assessments and guiding them through eligibility and application processes.
This position requires a balance of empathy, accuracy, and compliance, ensuring individuals receive timely and appropriate services while meeting all regulatory requirements.
Location: Greater Colorado Springs Area, hybrid schedule (in-office, remote, and in community setting)
Schedule: Monday - Thursday (7:30 - 5:30), Friday (7:30 - 12)
Pay: $22.32 - 23/18/hour, depending on experience
Key Responsibilities
- Conduct and document in-home functional needs assessments for applicable programs
- Complete and submit Medicaid, ARG, and long-term care waiver applications
- Maintain accurate and timely electronic records across multiple systems
- Review and analyze case documentation, including medical, behavioral, financial, and legal records
- Enter detailed case notes while maintaining compliance with HIPAA and regulatory standards
- Provide high-quality customer service to internal and external stakeholders
- Communicate effectively via phone, email, and virtual platforms
- Build and maintain positive relationships with community partners and stakeholders
- Stay current on long-term care, Medicaid, and related program regulations
- Participate in required meetings, trainings, and professional development
- Assist with departmental coverage and adapt to evolving regulatory requirements
Qualifications:
Minimum Requirements:
- Bachelor’s degree in social services or similar study OR 5 years of experience working with vulnerable populations, individuals with disabilities, or within human services/non-profit settings OR Equivalent combination of education and relevant experience
- Strong computer skills, including Microsoft 365, Outlook, and Adobe Acrobat
- Ability to learn multiple databases and systems
- Excellent written and verbal communication skills
- Strong organizational and time management abilities
- Ability to manage multiple priorities and deadlines
- Demonstrated empathy, cultural awareness, and emotional intelligence
- Ability to maintain confidentiality and adhere to HIPAA standards
- Valid driver’s license, reliable transportation, and required insurance
Preferred Qualifications:
- 1–3 years of case management experience
- Knowledge of Medicaid waiver programs and long-term care requirements
- Experience working with family systems, community resources, or service coordination
