Must Haves:
- Graduation from an approved nursing program
- Active RN license in the state of South Carolina
- Active BLS (AHA)
- 2 years of RN experience
Plusses:
- Previous rheumatology or nephrology experience
- EPIC experience
Day-to-Day:
The Ambulatory RN provides high‐quality, patient‐centered care to individuals receiving rheumatology and nephrology services in an outpatient clinic setting. This role supports providers, coordinates care across the continuum, and ensures safe, efficient clinical operations while delivering an exceptional patient experience.
Key Responsibilities:
• Perform comprehensive patient assessments, triage concerns, and manage symptom‐based protocols for chronic autoimmune and renal conditions.
• Support providers during exams and procedures, including medication administration, injections, infusion coordination, and diagnostic preparation.
• Educate patients and families on disease processes, treatment plans, lifestyle management, and medication adherence.
• Coordinate care across specialties, diagnostic services, and external providers to ensure continuity of care for complex chronic patients.
• Manage patient messages, refill requests, and follow‐up needs via the electronic medical record (EMR).
• Maintain accurate clinical documentation and uphold regulatory, infection control, and safety standards.
• Participate in quality improvement initiatives, workflow optimization, and population health monitoring.
• Collaborate closely with physicians, advanced practice providers, medical assistants, and administrative staff to support clinic flow and patient satisfaction.
• Monitor lab results, triage abnormal findings per protocol, and facilitate timely provider communication.
• Assist with prior authorizations, referrals, and coordination of specialty medications (including biologics).
