Position Overview
We are seeking a dedicated Licensed Vocational Nurse to join our field-based wound care team. In this role, you will provide hands-on wound assessment and treatment to patients across Riverside and San Bernardino Counties. You will work under the supervision of physicians and advanced practice providers, serving as a critical point of contact for patients managing acute and chronic wounds in post-acute and community settings.
Key Responsibilities
- Perform focused wound assessments, including evaluation of wound size, appearance, drainage, and signs of infection
- Execute wound care plans as ordered by supervising physicians or advanced practice providers
- Administer dressing changes, wound cleansing, packing, and routine wound treatments
- Deliver Ultramist therapy in accordance with established clinical protocols
- Identify and communicate changes in wound status to supervising providers in a timely manner
- Assist with advanced wound care procedures as directed
- Educate patients and families on wound care, treatment goals, and signs of complications
- Collaborate with physicians, NPs/PAs, RNs, and facility staff to coordinate patient care
- Maintain accurate, timely documentation of assessments, treatments, and patient progress in the EMR
- Upload wound photos, measurements, and clinical notes per organizational standards
- Represent Innovative Wound Solutions with professionalism and compassion in all patient and facility interactions
Qualifications
- Graduate of an accredited LVN program
- Active California LVN license in good standing
- Current CPR certification (AHA BLS)
- Minimum of one year of clinical LVN experience; post-acute or outpatient setting preferred
- Prior wound care experience preferred; Ultramist training will be provided
- Strong clinical documentation skills and attention to detail
- Excellent communication and interpersonal skills with patients, families, and interdisciplinary teams
- Ability to work independently in the field with minimal direct supervision
- Reliable transportation and valid California driver's license required
- Ability to travel throughout Riverside and San Bernardino Counties
