<p>We are seeking a Tuesday Night 12 HR LVN Per Diem Night Shift Available to join our team! You will be responsible for delivering high quality care to assigned patients.<br><br><strong>​Responsibilities:</strong></p>
<ul>
<li>Care for long term disabled population</li>
<li>Provide care to subacute population including trach and ventilator population</li>
<li>Medication pass and treatment nurse to 16 patients with feeding tube</li>
<li>Supervise and collaborate with CNAs on your shift</li>
<li>Document in EHR using PCC software</li>
</ul>
<p>​<strong>Qualifications:</strong></p>
<ul>
<li>Subacute facility experience prefered</li>
<li>Familiarity with trach/vent patients prefered</li>
<li>Excellent time managment skills required</li>
<li>Compassionate and caring demeanor</li>
<li>Excellent written and verbal communication skills</li>
<li>Ability to work independently and as a team required</li>
</ul>
