POTENTIAL FOR $5000.00 SIGN ON BONUS*

Elaine Ellis Center of Health is hiring a Licensed Professional Counselor to join our Mental Health and Substance Use Disorder team. Licensed Professional Counselors with SUD experience are welcome to apply. This position will provider services to both our DC and College Park. MD locations and has the potential for some remote work. EECH is a federally qualified healthcare center providing a wide array of healthcare services to the underserved population in Northeast, Washington DC and College Park. MD. Applicants must have licenses to practice in both DC and Maryland and have the desire to work with a challenging population.

SPECIFIC DUTIES AND RESPONSIBILITIES of this position:

· Conduct and document initial assessment of patient and treatment program readiness.

· Conduct and document on-going assessments of patient’s strengths, counseling needs and progress.

· Develop appropriate treatment plans and goals based on individual history and needs.

· Ensure an appropriate safety plan is established.

· Review and analyze treatment plans to evaluate progress.

· Collaborate with prescribing physicians and other health professionals regarding patient’s treatment and progress.

· Support patients, families and caregivers in self management, self efficacy and lifestyle and behavioral changes.

· Maintain a therapeutic relationship with patients to achieve desired goals.

· Abide by laws, regulations and standards of practice regarding ethics for the provision of treatment under the scope of license and certification.

· Develop aftercare programs and follow-up procedures when treatment is completed.

· Record and maintain patient information accurately and timely.

· Provide education to patients and community members.

· Refer patients to other providers and specialists as appropriate.

· Consult with and provide consulting services to other providers.

· Participate in provider meetings and other Center meetings as needed.

· Adhere to the Uniform Data System (UDS) productivity standards.

· Participate in 2 outreach events (Back to School and Thanksgiving) and offsite programs as assigned by the CMO.

· Attend and participate in teaching and general staff conferences.

· Actively participate in team training for patient centered care coordination.

· Continually update education, therapeutic techniques and methodology; apply new knowledge to your position.

· Perform other duties and assist with special projects as assigned.
