VNA of Hanover & Spring Grove – Home Health Department
Location: Hanover, PA | Schedule: Full-time | Reports to: Clinical Manager of Therapy Services

At the VNA of Hanover & Spring Grove, our mission is to provide compassionate, high-quality care to our community. We are currently seeking a skilled and dedicated Physical Therapist to join our Home Health team. In this role, you will provide vital rehabilitative care to patients in their homes, supporting independence, comfort, and quality of life.

Key Responsibilities:

  • Deliver comprehensive physical therapy services to Home Health and Hospice patients following a personalized plan of care.

  • Assess and evaluate therapeutic/functional status, including strength, mobility, gait, and range of motion.

  • Develop, direct, and adjust treatment programs based on patient needs.

  • Instruct patients, families, and caregivers on therapeutic techniques and equipment use.

  • Collaborate with physicians and interdisciplinary team members for coordinated, patient-centered care.

  • Supervise and guide Physical Therapist Assistants according to home health regulations.

  • Maintain thorough and timely clinical documentation in compliance with Medicare and agency standards.

  • Manage caseload efficiently, coordinate travel time for visits, and ensure cost-effective service delivery.

Qualifications:

  • Bachelor’s or Master’s degree in Physical Therapy from an accredited program.

  • Current Pennsylvania Physical Therapist license (or application in process).

  • Minimum of two (2) years of Physical Therapy experience (Home Health preferred).

  • CPR certification.

  • Valid driver’s license, reliable transportation, and automobile insurance required.
