Job Description

Job DescriptionSunnyside Manor is seeking part time LPN for the 7-3/3-11 shifts in our Assisted Living/Recollections neighborhood. We are currently seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team and provide exceptional care to our residents in accordance with New Jersey Nursing Standards. In this role, the LPN will deliver direct nursing services, including administering medications, monitoring resident conditions, assisting with activities of daily living, and supporting care planning and documentation. The LPN will also supervise and direct nursing assistants and home health aides, collaborate with interdisciplinary team members, and help maintain a safe, clean, and respectful environment that supports residents’ health, dignity, and quality of life. The ideal candidate will be licensed in New Jersey, possess excellent clinical and communication skills, demonstrate sound judgment, and have a passion for elder care. This position reports to the Charge Nurse/RN an Wellness Director.

Responsibilities

  • Provide direct nursing care to residents, including administering medications and treatments per physician orders.
  • Observe, assess, and document residents’ conditions and communicate significant changes to the appropriate healthcare professionals.
  • Participate in care planning and assist residents with activities of daily living (ADLs).
  • Supervise and direct nursing assistants to ensure resident care is carried out according to care plans and standards.
  • Maintain accurate medical records and ensure timely documentation of all clinical interventions and incidents.
  • Assist with resident admissions, discharges, and transfers, including assessments and data collection.
  • Communicate effectively with residents, families, physicians, and interdisciplinary team members to promote quality care.
  • Ensure resident rights, dignity, and confidentiality are respected at all times.
  • Participate in staff meetings, required in-services, and training to stay current with nursing standards and practices.
  • Respond to emergency situations promptly and appropriately, using sound clinical judgment.
  • Support quality assurance and performance improvement (QAPI) activities as assigned.
  • Maintain a clean, safe, and organized work environment, and follow all facility safety protocols.


Requirements
High school diploma or equivalent
Valid LPN Nursing License
Accept a background check to be performed
Flexible hours - nights/weekends
English
