LICENSED PRACTICAL NURSE - PART TIME

Job DescriptionFranklin Township Partnership Health Center is seeking a compassionate, organized, and patient-focused Licensed Practical Nurse (LPN) to join our healthcare team on a part-time basis (25 hours per week). This role is ideal for an experienced LPN who enjoys working in a collaborative primary care environment and is committed to providing exceptional patient care.

The LPN will support physicians and clinical staff by providing direct patient care, coordinating follow-up services, managing patient documentation, and assisting with chronic disease management programs. The successful candidate will demonstrate excellent clinical, communication, and organizational skills while maintaining a high standard of patient-centered care.

Responsibilities include:

• Obtaining full history on all patients, including medications, medical history, surgical history, social history and depression screening
• Assisting physicians with exams
• Ordering labs, testing and medications
• Holter monitor application
• Blood pressure monitoring
• Chronic care management

Schedule - 2 (two) - 10 hour days and every other weekend (9a-1p)

Qualifications

• Current New Jersey Licensed Practical Nurse (LPN) license in good standing
• Graduate of an accredited Practical Nursing program
• Prior experience in a primary care, family medicine, internal medicine, or outpatient setting preferred
• Strong knowledge of medical terminology and electronic medical record (EMR) systems
• Excellent communication, interpersonal, and organizational skills
• Ability to work independently and as part of a multidisciplinary healthcare team
• Proficiency in patient assessment and clinical documentation
