Our organization is in search of a committed Physical Therapist to deliver exceptional, patient-focused rehabilitation services within our Healthcare & Medical Services division. In this position, you will engage with a varied patient population across post-acute, skilled nursing, and other care environments, assisting individuals in regaining functionality, alleviating pain, and enhancing their overall quality of life.
Key Responsibilities
- Conduct comprehensive evaluations and create personalized, evidence-based treatment plans.
- Administer a wide array of therapeutic interventions, such as therapeutic exercises, gait and balance training, neuromuscular re-education, manual therapy, and education for patients and their families.
- Regularly assess patient progress, adjust care plans as necessary, and work collaboratively with an interdisciplinary team, which may include physicians, nurses, occupational and speech therapists, social workers, and other healthcare professionals to ensure cohesive and thorough care.
- Maintain accurate and timely documentation in the electronic medical record, complying with all regulatory, payer, and organizational standards. This includes evaluations, daily treatment notes, progress reports, discharge summaries, and participation in care conferences.
- Ensure adherence to federal, state, and professional regulatory requirements, including compliance with physical therapy practice standards and ethical guidelines.
