Social Services Assistant (Part-Time)

Job Summary:
At Hadley Pointe for Nursing Rehab and Care, we are dedicated to providing exceptional care to our residents while fostering a warm and supportive community. We are seeking a compassionate and organized Social Services Assistant to join our team. As a part-time member of our team, you will play a vital role in ensuring the social, emotional, and psychological well-being of our residents.
Responsibilities:

Provide emotional support and companionship to residents, engaging in activities such as conversation, games, and outings

Assist with daily living activities, including personal care and ADLs, under the supervision of licensed staff

Collaborate with the social services team to develop and implement individualized care plans

Facilitate resident activities, events, and group programs

Maintain accurate records and documentation, including resident assessments and service provision

Participate in team meetings and contribute to a positive and supportive work environment
Requirements:

High school diploma or equivalent required

Previous experience in a healthcare or social services setting preferred

Ability to work varying shifts, including weekends and holidays

Strong communication and interpersonal skills

Ability to maintain confidentiality and respect resident privacy

Adaptability and flexibility in a fast-paced environment
