\n\nRequired Qualifications — Home Health RN / Travel RN\n

- Active Registered Nurse (RN) license in New York (must have license in hand)\n
- Minimum 1 year recent adult Home Health or Hospice clinical experience as a Home Health Nurse\n
- Current CPR/BLS certification (AHA preferred)\n
- Proficiency with HomeCare HomeBase (HCHB) EMR (used within last 6 months or willing to retrain)\n
- Proficiency in OASIS documentation (used within last 6 months or willing to retrain)\n
- Reliable transportation, valid driver’s license, and current auto insurance\n
- Willingness to travel up to 75 miles from the home clinic to cover Westchester County\n
- Ability to complete clinical documentation within 24 hours of patient visit\n
- Open to on-call and weekend rotation as needed\n
- Cannot be a current direct employee of any Amedisys location\n
\n\nKey Responsibilities — Homecare Registered Nurse\n

- Provide skilled home nursing care per physician orders and agency policies as a Home Health RN\n
- Conduct 5–10 home visits per day (average 6–8 productivity points daily; ~30 points weekly)\n
- Perform start-of-care visits, comprehensive assessments, wound care, medication management, and skilled nursing interventions\n
- Supervise and evaluate Home Health Aides and LPNs/LVNs when assigned\n
- Maintain accurate, timely clinical documentation in HCHB and complete required OASIS assessments\n
- Collaborate with the interdisciplinary team to coordinate patient-centered care across home health services\n
- Participate in periodic office meetings and be flexible to float to nearby clinics if patient census requires\n
- Deliver compassionate, professional patient care in the community setting as part of our homecare team\n
