**Join Our Team as an Occupational Therapist in Home Health!**\n\n**Job Description:**\n\n- Are you ready to make a meaningful impact in patients' lives? As an Occupational Therapist, you’ll deliver exceptional care in the comfort of patients' homes.\n \n- **Position Type:** Full-Time, 1.0 FTE \n- **Schedule:** Typically Monday to Friday, 8 AM - 5 PM (some evenings and weekends may be required) \n- **Location:** Missoula, MT \n\n**Essential Duties and Responsibilities:**\n\n- Evaluate and establish a personalized plan of care to meet each patient's needs.\n- Coordinate with other disciplines to ensure comprehensive care and request additional services as needed.\n- Deliver direct patient care based on established care plans and physician orders.\n- Utilize your expertise in occupational therapy evidence-based practices and Medicare regulations during evaluations and treatment.\n- Collaborate with the Quality team to accurately complete required assessments and modify care as necessary.\n- Actively participate in multidisciplinary conferences, effectively communicate with physicians, and contribute to care planning.\n- Document all skilled services and relevant patient interactions in real-time.\n- Engage in Quality Assurance Performance Improvement initiatives to help the agency achieve its goals.\n- Maintain accurate time logs and mileage, adhering to agency policies regarding travel.\n- Communicate effectively with the scheduling team to optimize care continuity and travel efficiency.\n- Supervise and support Occupational Therapy Assistants and Certified Nursing Assistants in care delivery.\n- Stay updated on licensure requirements and engage in continuing education opportunities.\n- Uphold ethical standards of practice as set forth by the occupational therapy licensure board and agency policies.\n- Perform other duties as necessary to support the team and patients.\n\n**Minimum Qualifications:**\n\n- Bachelor’s degree or higher from an accredited Occupational Therapy program.\n- Current license to practice in the State of Montana.\n- Valid driver’s license and appropriate auto insurance.\n- A minimum of two years of previous OT experience is preferred.\n\n**Why Catalytic Solutions (CatSol):**\n\nAt CatSol, we connect dedicated therapists with rewarding assignments across the country.
