Location: Grand Junction, CO 81506
Shift: Variable Day Shift (12-hour shifts)
Schedule: 3 days/week (36 hours guaranteed)
Duration: 13 Weeks

Position Summary

A healthcare facility is seeking a dedicated Licensed Practical Nurse (LPN) with experience in Long Term Care (LTC) to support patient care in a skilled nursing environment. This role requires delivering high-quality care while maintaining compliance with healthcare regulations and standards in a fast-paced setting.

Key Responsibilities

• Provide direct patient care in a long-term care setting
• Monitor patient conditions and promptly report changes to the care team
• Administer medications and treatments as prescribed
• Maintain accurate and timely patient records and documentation
• Collaborate with physicians, registered nurses, and other healthcare staff
• Follow safety protocols and infection control guidelines
• Assist residents with activities of daily living (ADLs)

Qualifications

• Active LPN/LVN license required
• Previous experience in Long Term Care (LTC) preferred
• Strong clinical and patient care skills
• Ability to work 12-hour day shifts in a dynamic environment
• Excellent communication and teamwork abilities
