RN CASE MANAGER - HOME HOSPICE | DETAILS AND COMPENSATION:
- Location: Fond du Lac, WI – 100% On-site, field based work

- Coverage Area: Fond du Lac, Dodge, and Columbia counties, may include surrounding Wisconsin locations
- Payrate: $56.14/hr
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 4:30 PM

- Average of 5 patient visits per day
- No weekends or on-call
RN CASE MANAGER - HOME HOSPICE | SUMMARY AND HIGHLIGHTS:
The RN Case Manager will provide comprehensive, patient-centered care in a home and community-based setting. This role focuses on assessment, care coordination, and collaboration with interdisciplinary teams to ensure high-quality outcomes for patients with complex needs.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN CASE MANAGER - HOME HOSPICE | RESPONSIBILITIES:
- Perform comprehensive assessments of high-risk patients
- Develop, implement, and reassess individualized care plans
- Coordinate care transitions across healthcare settings
- Collaborate with physicians, social workers, and interdisciplinary team members
- Provide wound care and symptom management
- Educate patients, families, and caregivers on disease processes and end-of-life care when applicable
- Maintain detailed clinical, functional, and outcome documentation
- Advocate for patients while ensuring privacy and confidentiality
- Identify opportunities for health promotion and illness prevention
RN CASE MANAGER - HOME HOSPICE | REQUIREMENTS:
- Education: Associate’s or Bachelor’s Degree in Nursing
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Wisconsin RN license
- Wound care experience
- Valid driver’s license, reliable vehicle, and current auto insurance
- Current CPR/BLS certification
- Comfortable working in home-based care environments
- Familiarity with Home Care Home Base (HCHB) strongly preferred* - experience with MatrixCare or other EMRs is acceptable
- Case Management experience
- Hospice or home health experience*
- * Candidates without HCHB or hospice experience will receive training
Preferred Experience:
- Experience working with frail elderly populations
- Skilled Nursing, Assisted Living, or home care background
- Care planning, interdisciplinary collaboration, and documentation expertise
- Pain and symptom management, including end-of-life medications
