Responsibilities

• Provide emotional support and counseling to clients in need.

• Develop and implement individualized care plans for clients.

• Conduct assessments to identify client needs and resources.

• Engage in continuous professional development and training opportunities.

• Collaborate with healthcare professionals to ensure comprehensive client care.

• Participate in team meetings to discuss client progress and strategies.

• Monitor and evaluate client progress towards achieving care plan goals.

• Ensure compliance with all relevant legal and ethical standards.

• Foster a supportive and growth-oriented environment for clients and colleagues.

• Maintain accurate and up-to-date client records and documentation.

• Stay informed about community resources and refer clients as needed.

• Utilize data and metrics to track and improve service delivery.

Experience/Qualifications

• Licensed Clinical Social Worker (LCSW) certification in Colorado.

• Master's degree in Social Work or related field.

• Commitment to continuous professional development and learning.

• Minimum of 2 years experience in a healthcare or social work setting.

• Strong skills in developing and implementing care plans.

• Excellent communication and interpersonal skills.

• Ability to work independently and be accountable for results.

• Proven ability to collaborate with multidisciplinary teams.

• Proficiency in maintaining accurate client records and documentation.

• Familiarity with community resources and referral processes.
