We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The FNP will play a crucial role in providing comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic illnesses. Responsibilities
- Conduct thorough patient assessments and physical examinations to determine health status.
- Develop and implement individualized care plans based on patient needs and preferences.
- Manage chronic conditions and provide education on disease prevention and health maintenance.
- Perform medical procedures such as IV insertion, suturing, and basic life support as required.
- Maintain accurate medical records and documentation in compliance with healthcare regulations.
- Collaborate with interdisciplinary teams to coordinate patient care, including case management for complex cases.
- Provide specialized care for populations including infants, elderly patients, and those requiring hospice palliative medicine.
- Stay updated on best practices in dementia care and other relevant areas of family medicine.
Requirements
- Master’s degree in Nursing with a specialization as a Family Nurse Practitioner.
- Current state licensure as a Nurse Practitioner with prescriptive authority.
- Proficient in performing physical examinations and patient assessments.
- Experience in managing medical records effectively.
- Knowledge of basic life support techniques and certifications.
- Familiarity with dementia care practices is preferred.
- Experience in infant care is advantageous.
- Skills in hospice palliative medicine are a plus.
- Ability to perform IV insertion and suture procedures is desirable.
- Strong communication skills and the ability to work collaboratively within a healthcare team.
