Job Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The FNP will play a crucial role in providing comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic illnesses. Responsibilities

  • Conduct thorough patient assessments and physical examinations to determine health status.
  • Develop and implement individualized care plans based on patient needs and preferences.
  • Manage chronic conditions and provide education on disease prevention and health maintenance.
  • Perform medical procedures such as IV insertion, suturing, and basic life support as required.
  • Maintain accurate medical records and documentation in compliance with healthcare regulations.
  • Collaborate with interdisciplinary teams to coordinate patient care, including case management for complex cases.
  • Provide specialized care for populations including infants, elderly patients, and those requiring hospice palliative medicine.
  • Stay updated on best practices in dementia care and other relevant areas of family medicine.

Requirements

  • Master’s degree in Nursing with a specialization as a Family Nurse Practitioner.
  • Current state licensure as a Nurse Practitioner with prescriptive authority.
  • Proficient in performing physical examinations and patient assessments.
  • Experience in managing medical records effectively.
  • Knowledge of basic life support techniques and certifications.
  • Familiarity with dementia care practices is preferred.
  • Experience in infant care is advantageous.
  • Skills in hospice palliative medicine are a plus.
  • Ability to perform IV insertion and suture procedures is desirable.
  • Strong communication skills and the ability to work collaboratively within a healthcare team.
