Our client is seeking an experienced Registered Nurse Case Manager (RNCM) dedicated to providing comprehensive and personalized care to elderly patients. This full-time role is based in our bustling healthcare center, working directly with patients and their caregivers in a variety of settings including the clinic, adult day centers, patients' homes, and more.
Who You Are
You are a seasoned Registered Nurse who is passionate about geriatric healthcare, with at least 3 years of direct care experience. You understand the unique challenges of chronic condition management, inpatient transitions, and home visits. Your background includes working with MediCal populations and managing clinical skills such as IVs, blood draws, wound care, and medication management. You are a strong communicator who is comfortable coordinating care, managing schedules, and triaging in an outpatient setting.

Your Role
As an RNCM, you will be the "quarterback" for your panel, making critical contributions to interdisciplinary team meetings and driving forward participant care plans. Your responsibilities will include conducting face-to-face assessments, delivering and documenting nursing interventions, coordinating all aspects of care delivery, educating participants and caregivers, and assisting with wound care and other procedures. You will also be responsible for after-hour calls on a rotating schedule.
How You Qualify
You must be a graduate of an accredited school of nursing, with a BSN preferred. You must have a valid RN license and a minimum of 2-4 years of experience caring for medically complex or older adults. Case management experience is required. You should have strong clinical acumen in chronic disease management and geriatric care, and experience in managing clinical interventions.

The expected salary range for this position is $55.00 - $68.00/hour, with the final offer determined by your qualifications, skillset, and experience.
