What We're Looking for:
We are seeking dedicated individuals who are passionate about making a difference in the lives of older adults. As a Care Manager at ElderTree, you will play a crucial role in ensuring that our clients receive the highest quality of support.
Responsibilities include:
- Comprehensive Care Management: Oversee and coordinate tailored care plans, ensuring that our clients receive appropriate and timely services.
- Client Advocacy: Serve as a strong advocate for clients, ensuring their needs are met in home, medical and care facility settings, including while communicating with healthcare providers.
- Support and Collaboration: Work closely with team members to address clients’ needs, and contribute your expertise to the team, helping to enhance the collective knowledge and skills within ElderTree.
- On-Call Availability: Participate in after-hours or weekend on-call rotations as needed to provide continuous support to clients.
- Field Work and Documentation: Update family members and other members of the care team, and maintain thorough and accurate documentation. Approximately 90% of the role involves field work, with 10% dedicated to follow-up and office-based tasks.
Qualifications and Requirements:
- Associates' or Bachelor’s degree
- Available 9 am to 6 pm Monday-Friday and some weekends.
- Dependable, adaptable, and accountable, with a strong commitment to providing exceptional care and support to clients.
- Must be able to lift and maneuver up to 50 pounds.
- Must be comfortable and willing to drive regularly around the Virginia and DC Metro area
Preferred Qualifications:
- Ideal/preferred candidates from a variety of backgrounds, including RN, OT, PT, speech pathologist, nutritionist, and social worker
- A Master’s degree in a related field
- 4 years of relevant experience in gerontology, mental health, home care, or similar
