Pinnacle Hospice is seeking an After-Hours Hospice LVN to provide skilled, on-call nursing care outside of standard business hours, throughout the Sacramento Area (ROSEVILLE, ROCKLIN, AUBURN, LINCOLN, FOLSOM).

Qualifications

• Current LVN license in good standing
• Strong clinical judgment and ability to work independently
• Excellent communication and problem-solving skills
• Compassionate presence in emotionally sensitive situations
• Valid drivers license
• Reliable, insured vehicle in good working condition (per state and organizational requirements)

Responsibilities

• Hospice Afterhours RN/LVN (per diem/oncall)
• Be there when patients and families need you most
• Pinnacle Hospice is seeking an After-Hours Hospice LVN to provide skilled, on-call nursing care outside of standard business hours
• The After-Hours Hospice LVN provides professional nursing care to hospice patients during evenings, nights, and weekends
• This role ensures continuity of care, symptom management, and emotional support for patients and families from 5:00 PM - 8:30 AM
• The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care
• Respond to after-hours patient calls and urgent care needs
• Perform skilled nursing assessments and symptom management
• Provide emotional support and education to patients and families
• Implement and update patient plans of care as needed
• Communicate patient condition changes to the interdisciplinary team
• Collaborate with physicians and caregivers regarding patient needs
• Complete accurate and timely clinical documentation
• Uphold hospice philosophy, policies, and regulatory standards
