Position: Case Manager II-III CFC [1154]
Program: Amarillo, Texas – CFC (Community First Choice) Personal Assistance Services/Habilitation (PAS/HAB)
Salary range:
Case Manager II: $43,305.60 Annually
Case Manager III: $46,009.60 Annually
Shift available: Full-time

Essential duties and responsibilities:
- Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
- Responsible for provision of service coordination including completing Community First Choice Assessments, Individual Disability Related Conditions Assessments, and Coordinating with Managed Care Organizations and consumers
- Will participate in the development and implementation of departmental guidelines and procedures
- This position may be assigned to serve in the Texas Home Living (TxHmL), General Revenue, and Home and Community-based Services (HCS) Programs
- Adheres to TPC Business Code of Conduct and reports concerns if ethical and/or business standards defined in the code are compromised
Required qualifications to be successful in the job:
- Bachelor's degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, criminal justice, or other related human services field
- For Case Manager II, minimum one (1) year of experience working directly with individuals with ID or DD
- For Case Manager III, minimum two (2) years of experience working directly with individuals with ID or DD
- Graduate work may be substituted for up to one (1) year of experience
- Good computer skills including Microsoft Word and Outlook
- Good writing and verbal skills

- Must have a valid Texas driver’s license and be insurable under agency vehicle policy
- Must be 18 years of age or older
Preferred qualifications to be successful in the job:
- The ability to organize documents for assigned caseload and meet time deadlines
- Ability to speak Spanish
