Field Eligibility Representative Job Description
Position Overview
The Field Eligibility Representative is a vital, field-based member of the HSS team responsible for guiding patients through the Medical Assistance (MA) application process. This role bridges in-person outreach with administrative follow‐up by screening patients for eligibility, collecting essential documentation, and coordinating with internal case management. The ideal candidate is proactive, organized, and able to work both independently in the field and collaboratively with office staff.
Key Responsibilities
- Patient Outreach & Screening:
- Visit hospitals, patient residences, or community locations to initiate MA application processing.
- Conduct in-person eligibility screenings using established federal guidelines and HSS protocols.
- Locate (if necessary) and contact all referred patients to verify eligibility and explain the MA application process.
- Documentation & Case Coordination:
- Collect and verify essential documents (e.g., birth certificates, bank statements, work verifications, IDs, Social Security cards) needed for MA applications.
- Assist patients in obtaining missing documents.
- Maintain and update accurate records in the HSS database as new information becomes available.
- Field & Office Integration:
- Provide regular updates to HSS Case Managers regarding case progress, challenges, and successful completions.
- Attend weekly in-office sessions in Baltimore for paperwork submission, case reviews, and database entry as required.
- Additional Duties:
- Manage a dynamic caseload, often with over 50 active cases, while prioritizing time-sensitive tasks.
- Adapt to evolving procedures and regulatory guidelines to ensure continuous process improvement.
Essential Qualifications
- Bilingual in Spanish and English is a plus but not required.
- High School Diploma or equivalent work experience
- Proven experience in field work, case management, or a customer service environment—preferably in healthcare or social services.
- Effective communication, time management, and organizational skills.
- Proficient in computer use and data entry.
- Ability to work effectively under pressure and handle unexpected challenges.
- Must possess a valid driver’s license and reliable transportation for field visits.
Work Hours and Employment Details
- Schedule:
- Part-time (with possible Full-time) position with flexible hours to accommodate patient and case needs.
- Combination of fieldwork and office duties; hybrid; office attendance is required weekly.
- Compensation & Benefits:
- Compensation structure is for the first 90 days there is a base pay of $600 per week with $50 - $75 per case rate. Thereafter there is no base pay, and it is $100 - $150 per case rate with per-case incentives.
- Location:
- Primary work locations include assigned field areas within Maryland and the HSS office located in downtown Baltimore (1020 Stiles St., Baltimore, MD 21202).
