Job DescriptionSalary:

TITLE: Associate Professional (AP)-Community Support Team

Reports To: CST Team Lead

Purpose: Staff providing Community Support Team Services consist of mental health and substance abuse rehabilitation interventions necessary to assist adults (age 18 and older) in achieving rehabilitative and recovery goals. This is an intensive community rehabilitative service that provides treatment and restorative interventions.


Education/Experience/Qualifications

  • Graduate of a college or university with an advanced degree in a related human service field and having less than 1 year of full-time, post graduate accumulated MH/DD/SAS experience with the population served, or a substance abuse professional who has one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling or
  • Graduate of a college or university with a Baccalaureate degree in a human service field and having less than 2 years full-time post bachelors degree accumulated
    MH/DD/SAS experience with the population service, or a substance abuse Professional with less than 2 years experience of full-time, post-bachelors degree supervised experience in alcoholism and drug abuse counseling; or
  • Graduate of a college or university with a Baccalaureate degree in a non-related field and having 4 years of full-time, post-bachelors degree MH/DD/SAS experience with the population serviced, or a substance abuse professional who has four years of full-time, post-bachelors degree accumulated supervised experience in alcoholism and drug abuse counseling; or
  • Graduate of a college or university with a bachelors degree in a field other than human services with less than four years of full-time post-bachelors degree accumulated MH/DD/SAS experience with the population served, or a substance abuse professional with less than four years of full-time post bachelors degree accumulated experience in alcoholism and drug abuse counseling; or
  • A registered Nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing with less than four years of full-time accumulated experience in MH/DD/SAS with the population served.

Duties and Responsibilities

  • Assistance with therapeutic interventions to rehabilitate:

Functional skills

Adaptation, socialization, relational, and coping skills

Self-management of symptoms

Daily and community living skills

Behavior and anger management skills


  • Assumes the role of advocate, broker, coordinator, and monitor of service delivery system on behalf of the consumer.
  • Coordination of movement across levels of care.
  • Coordinate discharge planning and community re-entry following hospitalization, residential services and other levels of care.
  • Must be willing to provide services in various environments, such as homes, schools, jails, homeless, shelters, street locations, etc.
  • Therapeutic mentoring that directly increases the acquisition of skills needed to accomplish the goals of the PCP
  • Psycho-education and training of family, unpaid caregivers, and others who have a legitimate role in addressing the needs identified in the Person Centered Plan.
  • Direct preventive and therapeutic interventions that will assist with skill building.
  • Relapse prevention and disease management strategies.
  • Ongoing symptom monitoring and management
  • Ongoing medication monitoring, with reports to medical providers.
  • Service coordination activities within the established Person Centered Plan.
  • Input into the Person Centered Plan modifications.
  • Assist with ensuring compliance to all applicable rules, regulations, and policy and procedures of the agency and governing standards.
  • With support the guidance directs and participates in policy and program implementation. Assists with meeting deadlines.
  • Assist with working with the mental health agency to solve day to day problems regarding clients and/or their families.
  • Assist with auditing clinical charts to ensure completeness accuracy and regulatory compliance.
  • Assist with providing On-Call support under the supervision of the QP.
  • Maintain ongoing communication with agency stakeholders such as DSS, DJJ and LME, etc.
  • Ensure all paperwork is submitted in a timely manner.
  • Assist with implementing, monitoring and participating in quality assurance and improvement programs.
  • Participate in treatment team meetings along with other team members.
  • Performs other duties as assigned or directed.
