Key Responsibilities

  • Conduct thorough patient evaluations, including assessments of musculoskeletal, neuromuscular, and functional capabilities to identify tailored treatment requirements.
  • Create and execute patient-focused care plans that integrate therapeutic exercises, manual therapy, gait and balance training, functional mobility training, and pain management strategies, following physician directives and established clinical guidelines.
  • Regularly evaluate patient progress, adjust treatment plans as necessary, and provide clear guidance to patients, families, and caregivers on home exercise routines, safety measures, equipment usage, and methods to sustain functional improvements.
  • Work collaboratively with an interdisciplinary team, which includes physicians, nurses, occupational and speech therapists, social workers, and other healthcare professionals, to coordinate care, assist with discharge planning, and ensure optimal clinical outcomes.
  • Accurately document all assessments, goals, interventions, and patient responses in the electronic health record, complying with regulatory, payer, and organizational standards.

Working Conditions

In line with our organization’s inclusive culture that emphasizes dignity, respect, and professional growth, you will help foster a supportive and nurturing atmosphere for patients, families, and colleagues. Participation in clinical meetings, in-service training, and ongoing educational programs will be expected to maintain and enhance your clinical skills, uphold evidence-based practices, and support quality improvement initiatives.

Mentorship and Guidance

You may also provide mentorship to Physical Therapist Assistants and rehabilitation aides, delegating appropriate tasks while ensuring compliance with the established care plan.
