This per diem Nurse Practitioner role provides the opportunity to support hospice patients and families through advanced clinical care delivered where and when it is needed most. With flexible scheduling, this position allows you to conduct face to face visits, provide symptom management support, and collaborate closely with physicians and interdisciplinary team members to ensure timely, compassionate care. You will serve Broward County patients while remaining connected to a mission-driven clinical team focused on comfort, dignity, and quality of life.

Overview

The Nurse Practitioner (NP) provides clinical support to hospice patients and families across multiple care settings. The NP collaborates with the Interdisciplinary Group (IDG), hospice physician, and attending providers to deliver high-quality, goal-concordant care. Responsibilities include face-to-face visits, routine and urgent symptom management, clinical education, and participation in certification processes. Prescriptive authority and advanced clinical judgment are exercised in accordance with state regulations and organizational protocols.

Key Responsibilities

  • Conduct comprehensive face-to-face encounters for patients entering or continuing the third and subsequent benefit periods.
  • Perform evaluation, consent, and admission visits; assess patient eligibility and collaborate with the interdisciplinary team to initiate hospice services.
  • Perform routine, urgent, or symptom management visits as indicated by patient condition and care plan.
  • Assess, plan, and evaluate care in collaboration with the hospice physician and attending providers.
  • Participate in IDG meetings and collaborate with the interdisciplinary team to develop and revise patient plans of care.
  • Provide education to patients, families, and caregivers regarding disease progression, medication use, and end-of-life expectations.
  • Offer clinical consultation and mentorship to team members and participate in internal training activities.
  • Document all patient encounters and clinical interventions timely and in accordance with regulatory and policy standards.
  • Provide on-call services, including evenings and weekends, as assigned to support continuity of care.
  • Maintain clinical knowledge in palliative care, symptom management, pharmacology, and relevant state practice regulations.
  • Ensure compliance with HIPAA and hospice policy in all documentation and communication.
  • Collaborate with physicians and clinical leadership to ensure appropriate prescribing practices and care planning.
  • Practice in accordance with scope authorized by the state, including physician collaboration where required.
  • Performs other duties as required.

Qualifications

  • Graduate from an accredited Baccalaureate School of Nursing and an NP-focused Master's or Doctoral Nursing Program.
  • Current RN license and Advanced Practice Nurse license in practicing state.
  • Active prescriptive authority and DEA registration, as required by state law.
  • Minimum two (2) years of experience as a Nurse Practitioner; hospice or palliative care experience preferred.
  • Comprehensive knowledge of palliative care, pharmacology, and end-of-life symptom management.
  • Effective communication and interpersonal skills with patients, families, and interdisciplinary team members.
  • Palliative certification (CHPN, ACHPN) preferred but not required.
  • Reliable transportation, valid driver's license, and current auto insurance.

The starting compensation for this position is shown below. Arc Hospice is committed to transparency and equity in all compensation practices. The rate displayed reflects the starting hourly or salaried pay for the role and does not represent a range.
