Position Overview
We seek a dedicated Home Health Intake and Staffing Coordinator to manage patient referrals and facilitate smooth transitions into our Home Health programs. This role requires someone who is organized, compassionate, and able to coordinate effectively with healthcare providers, patients, and families to ensure all necessary documentation is completed accurately and efficiently.

Responsibilities

• Process new patient referrals and request / follow up on required clinical information/document

• Communicate with referral source, healthcare providers, patients, and families

• Schedule appropriate care staff based on patient needs and orders

• Ensure compliance with healthcare regulations and company policies

• Track referral sources and assist with relationship management

Qualification

• High school diploma required; Associate's degree in healthcare or related field preferred

• Experience in healthcare administration, especially in home health settings

• Knowledge of medical terminology and insurance verification processes

• Excellent organizational and time management skills

• Strong communication and interpersonal abilities

• Proficiency with electronic health records and office software - training in working with Kinnser EMR is preferred.

Schedule

• Monday to Friday (weekend as needed)

Medical Specialty Home Health experience preferred
