POSITION SUMMARY

Unity Health Care is seeking a dedicated Nurse Practitioner to join our clinical team. Under the supervision of the Medical Director, the Nurse Practitioner provides comprehensive care to patients by independently assessing, diagnosing, and treating health concerns, while promoting wellness through preventive services.

KEY RESPONSIBILITIES

  • Deliver preventive, diagnostic, and therapeutic care to patients.
  • Conduct physical exams, obtain medical histories, and perform evaluations.
  • Order and interpret diagnostic tests including labs and radiology.
  • Diagnose conditions and prescribe medications, including controlled substances.
  • Counsel patients on health maintenance and chronic disease management.
  • Make referrals to specialists, shelters, and hospitals when appropriate.
  • Provide emergency care and coordinate transportation as needed.
  • Collaborate with interdisciplinary teams including physicians, social workers, and clerical staff.
  • Participate in peer reviews, quality improvement efforts, and screening programs.
  • Attend team meetings and fulfill on-call responsibilities as required.

QUALIFICATIONS

  • Active DC RN license with certification by the ANCC or AANP.
  • Valid DEA and CDS licenses.
  • Minimum of 2 years of experience in an ambulatory care setting, preferably with homeless or underserved populations.
  • Willingness to participate in on-call rotations.

REQUIRED SKILLS

  • Strong knowledge of clinical practices, procedures, and patient care standards.
  • Ability to work both independently and collaboratively within a care team.
  • Commitment to providing high-quality, compassionate care to vulnerable populations.
