Job Overview
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will possess a strong understanding of anatomy and physiology, along with experience in various settings such as home health, outpatient, and inpatient care. As a Physical Therapist, you will play a crucial role in assessing patient needs, developing personalized treatment plans, and guiding patients through their rehabilitation journey to enhance their quality of life.

Responsibilities

• Conduct thorough evaluations of patients' physical abilities and limitations.
• Develop individualized treatment plans based on patient assessments and goals.
• Implement therapeutic interventions to improve mobility, strength, and overall function.
• Educate patients and their families on treatment protocols and home exercise programs.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate documentation of patient evaluations, treatments, and progress notes.
• Participate in discharge planning to ensure continuity of care post-treatment.

Requirements

• Valid Physical Therapist license in the state of practice.
• Strong knowledge of anatomy, physiology, and medical terminology.
• Experience in home health, clinic, inpatient, or outpatient settings is preferred.
• Excellent patient care skills with a compassionate approach to rehabilitation.
• Ability to develop effective discharge plans for patients transitioning from therapy services.
• Strong communication skills for interacting with patients, families, and healthcare teams.
• Commitment to ongoing professional development and staying current with industry practices.
• Graduated students encouraged to apply.
