As we enter a growth phase, Talem Home Care of Colorado Springs is hiring Part-Time PRN Registered Nurses to join our team. Our mission goes beyond offering patients a better life through compassionate care and companionship. We are committed to providing sincere, one-on-one care for our clients and their families, and a balanced work environment where our Registered Nurses have options. We are looking for PRN RNs to perform site visits for our clients on the Medicaid In-Home Support Services (IHSS) program. Appointments involve visiting the client's home during a caregiver's shift, reviewing the client's care plan with the client and caregiver, identifying if any changes need to be made in the care plan, and certifying that the caregiver is proficient in the performance of all IHSS-related tasks.

Job Responsibilities:
- Provide nursing support through all phases of care, taking into consideration age and developmental needs
- Support care plans and collaborate with the physician and healthcare team
- Perform basic administrative duties such as client assessments to ensure good health
- Educate and instruct the patient, patients’ family, and other staff as required in meeting nursing and related needs
- Supervise and teach caregivers as required for patient care according to law and regulations
- Notify patients’ physician and responsible staff of significant changes promptly
- Coordinate all services provided and consult with other health professionals as needed
- Effectively plan, organize, and set priorities, and complete work assignments with minimum direct supervision
- Perform regular visits according to the plan of care and submit complete visit notes within 24 hours of completion visit
Successful Candidates Must Have:
- Valid Colorado RN License
- Two years of experience in a home-care setting preferred
- Valid driver's license and access to reliable transportation
- Some experience/exposure to Colorado State Medicaid programs is preferred
- Completion of the appropriate twenty-hour training program or successful completion of skills validation via direct observation of the performance of tasks
- A working knowledge of geriatric care
- Bilingual in Spanish and English preferred but not required
- Strong interpersonal and communication skills and the ability to work effectively with the team, patients, and their families
- Problem-solving skills and ability to multi-task
